What is an Advance Care Plan?
Advance Care Plan (ACP) is a voluntary process to discuss and share your health-care preferences with your family and healthcare providers. The ACP process guides physicians, patients and their loved ones in making decisions based on the patient's values, beliefs, wishes and personal goals of care.

Answer a series of questions that will create a list of your future health and care preferences.
It is recommended that you complete your ACP in the presence of your Nominated Healthcare Spokesperson so that you can discuss and make them aware of the decisions to be made.
