What is a Life Plan?
A Life plan is a person-centred plan, based on that's important to PwDs. The plan shows the present life of the PwDs and the goals he/she wants to achieve in life. This can be facilitated and drawn out together as a guide to help you and PwDs prioritise PwDs life goals.
PwDs can have a voice in his/her life plan too! Start taking small steps to help you and PWDs identify and work on the goals now.
Why is having a Life Plan beneficial?
A life plan is personal to each individual. This can be facilitated and drawn out together as a guide and helps you to prioritise your life goals. Needs and aspiration are evolving and periodic assessment is needed. Start taking small steps to work on the goals.

Creating a life plan for PwD involves a few key steps

Knowing the PwD
Understand the PwD's abilities, interests, circle of support and vision for the future. In his/her view, what do people appreciate about him, what is important to him and how he would like to be supported.

Set goals
Work together to identify short-term and long-term goals based on interest and aspirations and how he and his circle of support can do to achieve these goals.

Develop a Life Plan
Make an appointment with us for a Family Envisioning Meeting (FEM) session. It will be an interactive 1 to 2 sessions with PwD and family members to outline training plan and actions needed to achieve the identified goals and create a life plan.

Implement the Plan
Start working towards the goal, breaking down tasks into manageable steps, and providing necessary support and guidance along the way.

Monitor progress
Needs and aspirations are evolving. Regularly review the progress, adjust the plan as needed, and celebrate achievements.

As the PwD progresses, plan for transitions to different life stages or settings, ensuring continuity of support and opportunities for growth and independence.

Our Lives Our Voices (OLOV) is a self-advocacy programme that empowers and supports youth and young adults with PWDs to share about themselves.

FAQ
Typically, a life plan can include information about self, review of current life such as level of independence, usual routine, health status, beliefs, circle of support. It will also include visions and goal, and actions that can be taken to achieve the goals.
Seek and enquire from your PWD's school or centre for staff/social worker trained in person-centred planning. They will be able to direct and support you in this area.
